I understand that I am required to attend all classes, rehearsals and performances that are scheduled for me. Three (3) additional rehearsals outside of my regular classes for Levels 6/7 students only, may be scheduled between April 6 and May 11, 2024, with least a one-week notice given. From May 13-19, 2024, All Intensive Program students may have daily class and rehearsals/performances.

I understand that if I miss 2 or more classes and rehearsals, it might be necessary to adjust my role or forfeit my role in the performances to accommodate my lack of class time and rehearsal, and in fairness to my classmates. This judgment will be made by the artistic faculty of the Tulsa Ballet Center for Dance Education.

I also understand that I will be required to attend all of my scheduled performance dates, between May 17-19, 2024. Final schedules will be provided closer to performance day, but I will not schedule conflicting activities on these days, without prior permission from the School Principal.

I understand that I will be responsible for providing for my student: class shoes, back-up pair of tights for all ballet students, and hair supplies for the production (in line with the TBCDE dress code).

I will submit this form and pay the End of Year Performance and Costume fees for a total as listed below. The performance fee portion will help cover some of the costs of: facility rental stagehands, lighting design, ticketing, publicity, printing, choreography and music. The costume portion covers the costumes students will wear during their pieces. Some costume fees will be for costume rentals since some pieces will belong to Tulsa Ballet, and will be returned after the final performance.

I understand that I will be responsible for purchasing all tickets I wish to receive for my child's performances.
